The Surry Transition Project (STP)

SCOSAR provides a variety of services, including employability skills-focused training, violence and recidivism prevention classes, life skills classes, substance use education, substance use disorder screening, assessment, treatment and recovery maintenance. Coordinated re-entry transition coaching and assistance, transportation, and Recovery to Work (RTW) services are also available.  These services are delivered within and after release from the Surry County Detention Center.  

Background
 

Revolving doors not only strains the resources of the criminal justice system but also jeopardizes public safety and hinders individual recovery. To address this complex challenge, the Surry Transition Project was implemented in the Surry County Detention Center in early 2025, with funding from the Comprehensive Opioid, Stimulant, and Substance Use program. The project is designed to interrupt the cycle of recidivism and reduce overdose incidents by focusing on early identification, comprehensive assessment, and treatment


The need for this intervention is further supported by the recognition that successful reentry into the community is crucial for sustained recovery and reduced recidivism. Therefore, the Surry Transition Project extends its support beyond the detention center walls by facilitating seamless transitions to community-based care. This approach acknowledges that addressing substance use disorders requires a continuum of care that begins within the criminal justice system and continues in the community.

Intake and Assessment Phase

During the first phase therapeutic process, structure, policies and procedures will be discussed.  We will also explore experiences surrounding the presenting problem(s). The initial evaluation may last 1-2 sessions. During this assessment phase, the clinician or SCOSAR staff member will be get to know the detainee.  SCOSAR staff will ask questions to gain an understanding of your worldview, strengths, concerns, needs, relationship dynamics, etc.  During this relationship building process, SCOSAR staff will be gathering information to aid in the therapeutic approach best suited for individual needs and goals.

Goal Development

After gathering background information, detainees and SCOSAR staff collaborate to identify therapeutic goals and possible pre-release concerns and needs.

Intervention Phase

This phase occurs anywhere from session two until graduation/discharge/termination.  Each detainee must actively participate in individual and/or group therapy sessions, utilize solutions discussed, and complete assignments between sessions.  Progress will be reviewed, and goals adjusted as needed.

Pre-Release Planning and Exit

As individuals make progress and approach goals, we work together to outline a pre-release plan essential for your reintegration into the community. Our Program Staff is here to offer programming, services, and support for up to six months following release from the detention center.

The Concept

The Surry Transition Project emphasizes an Detainee Honor Code to foster trust, accountability, and respect among detainees participating in substance use counseling services. This code outlines core principles such as honesty, respect, confidentiality, accountability, and commitment to recovery, as well as expectations of conduct and consequences for violations. 


Furthermore, the Surry Transition Project Job Readiness Workshop is designed to equip inmates with essential skills needed to successfully enter the workforce. During the workshop, inmates learn essential skills such as how to fill out a job application correctly, effectively answer common interview questions, identify key skills that employers seek and create a resume. Upon completion of the workshop, inmates receive a certificate acknowledging participation and skills acquired. 
 

A Basic Flagger Training is designed to provide an understanding of road flagging procedures and safety protocols. During the training, inmates learn how to be a safe & effective flagger, understand why proper flagger operations are important, identify proper flagging signals & procedures and learn standard flagger practices for various situations. Upon completion of training and to receive certification, the detainee must demonstrate flagging procedures and pass a 20-question assessment. The certification is NCDOT approved. 

The Surry Transition Project stands as a model for how bold, innovative action can reduce overdose fatalities, lower recidivism, and secure the health and safety of communities. By focusing on early identification, evidence-based treatment, and seamless transitions from custody to community-based support, the program embodies the comprehensive, coordinated approach required to confront the scourge of illicit drugs and addiction. Surry County is building a safer, healthier future for all its residents through relentless collaboration and a commitment to results.
